Lakes Regional Healthcare home care services are provided to people in Dickinson County in the comfort of their own homes. Services include skilled nursing, home care aides, physical therapy, occupational therapy, speech therapy, wound care and homemaker services. All services are provided under the direction of a physician’s plan of care. 

Skilled nursing services are provided for the treatment of acute or chronic illnesses. Nursing care may include patient assessments, medication setup, infusion therapy, cardiac and respiratory monitoring, dressing changes and other procedures. As our patient, nurses teach you about your illness, medications and how to best care for yourself. 

Home care aide services are provided to assist you with activities of daily living. They may include personal care, exercise assistance, light housekeeping and laundry. 

Therapy services are provided to patients for rehabilitation after an illness or injury. Therapists also provide education on what changes you can make in your daily life and living environment to live independently. 

Transition of Care Program

If you are in the hospital and not ready for home care to return home safely, our Transition of Care Program can help. Designed to improve your independence and confidence, the program provides goal-oriented, time-limited and therapy-focused care to help you at the end of your hospital stay. You are assessed for the program while you are still an inpatient at the hospital. If you meet the criteria, a Transition of Care home visit is made within 24 to 48 hours of discharge which will focus on the following: 

  • Physician/provider appointment importance
  • Review of “teach-back” education
  • Medication reconciliation
  • Assessment of compliance with self-management
